硝子体・白内障同時手術後の角膜形状に影響を及ぼす因子の検討

Factors influencing the corneal topography following simultaneous vitrectomy and cataract surgery

　硝子体・超音波水晶体乳化吸引・眼内レンズ移植同時手術（トリプル手術）を行った55眼での角膜形状変化を，24週以上にわたって検索した。38眼ではSF₆ガス置換術を併用した。術後の惹起角膜乱視量は経時的に減少した。多変量解析で，術後2週の角膜惹起乱視と相関のある因子は，ガスタンポナーデ，前房のフィブリン析出，虹彩後癒着であった。ガスタンポナーデ施行群の術後2週の惹起角膜乱視量は，非施行群よりも有意に大きかったが，術後12週以降では有意差はなかった。

We evaluated 55 eyes which received simultaneous pars plana vitrectomy, phacoemulsification-aspirationand intraocular lens implantation. Thirty-eight eyes received gas tamponade using SF₆. We tried to identify factors thatinfluence postoperative changes in the corneal shape up to 24 weeks. The corneal astigmatism gradually regressed aftersurgery. Multiple regression analysis showed three factors that are significantly correlated with postoperative astigmatism: gas tamponade, fibrinous exudation in the aqueous, and posterior synechia. Eyes with gas tamponade had significantlylarger astigmatism at 2 weeks after surgery than those without (p=0.002). There was no difference between the twogroups after 12 weeks of surgery.
