Familial juvenile nephronophthisis （Fanconi）を合併したamaurosis congenita （Leber）の1家系

Hereditary syndrome of amaurosis congenita (Leber) and famiiial juvenile nephronophthisis (Fanconi)

緒言

　先天失明を伴う網膜変性は，1867年Leber^1）が初めて報告した疾患でamaurosis congenita ofLeberと呼ばれている。本症は外国では比較的多く報告されているが，わが国では報告^2,3）がきわめて稀であり，家族性におこつたものは1例も報告されていない。本症の合併症は多くは中枢神経系におこつているが，1961年Senior^4）らは従来知られていなかつた事実として本症とfamilialjuvenile nephronophthisisとが合併した1家系を報告した。以来同様な症例がいくつか報告^5〜15）され，現在では両者は一つの症候群としておこるものとみなされている。本邦では篠田^2）らが1例を報告している。今回われわれは同胞6名中3名（2名は直接検査，一名は家族歴から）にfamilialjuvenile nephronophthisisと典型的なamau—rosis congenitaとが合併しておこつた1家系を経験した。他の同胞2名には，遠視性弱視が認められた。同胞の母親は全身的には正常であり，眼機能検査でも著変はみられなかつたが眼底赤道部に軽度の網膜の変性を思わせる所見が認められた。

A family is described in which three of six sib-lings suffered from amaurosis congenita (Leber) in conbination with congenital type of familial juvenile nephronophthisis (Fanconi). Two of the three cases were examined by the authors and the diagnosis was confirmed. The last case was examined by records. She had congenital blind-ness and died by a convulsion-attack at her one and half years of age. The two cases exa-mined by the authors had congenital blindness with nystagmus but showed least changes in the fundus by ophthalmoscopy. They died by uremia at their one and two years of age, respect-ively.
