視交叉付近腫瘍症例と視野

Clinico-statistical Evaluation on the Visual Field Changes in Cases of Tumors of Chiasmal Regions

I．緒言

　神戸大学眼科において開頭した視交叉部付近原発の腫瘍約200例のうち，昭和23年以後20年間の統計に供し得た179例を対象として，術前の視野に焦点をしぼつて視野障害の起こる順序，方向の他，視野障害が直接神経線維に加わる圧迫によるものか，あるいは血行障害に起因するものかという疑問をかかげ，主として下垂体腺腫，頭蓋咽頭腫，髄膜腫の3つの性質の異なる腫瘍について視野の観察を行なつた。

One hundred and seventy-nine cases of tumors of the chiasmal region were craniotomized in the Eye Clinic of Kobe University from 1948 to 1968. The author studied precisely the visual fields of these cases.

Classification of tumors in the present study were as follows : pituitary adenomas 83 cases, craniopharyngiomas 63 cases, meningiomas of the tuberculum sellae 26 cases, meningiomas of the sphenoid wing 3 cases, ependymomas 2 cases and each one case of ectopic pinealoma and chordoma.

In more than 50% of these patients, remark-able arachnoideal thickening was observedmacroscopically and histologically.
