Eye Symptoms in Cryptococcal Meningitis

緒言

　Cryptococcal meningitisについては，1894年Bussueをはじめとして，わが国でも渡辺，五斗以来幾多の報告がみられる，本症はかつては，はなはだしく予後不良の疾患であつたが，1961年Amphotericin Bが使用され始めてからその死亡率は半減した，しかし，命をとりとめても脳神経障害，特に視機能障害が残されて，難治の場合が多い。

　われわれは1967年から1974年6月までの約5年間に，Cryptococcal meningitis 7例を経験した。このうち3例はすでにクモ膜所見等について報告したが，その後さらに4例を経験したので，計7例（開頭例7例と非開頭例3例）について，眼科的症状とクモ膜病理，後遺症に対する治療について検討し報告する。

Seven cases suffering from cryptococcal me-ningitis were reported. One of which was dead by the general cryptococcosis. Remaining six patients complained of visual disturbances dur-ing their course. In two cases, visual acuity became normal with conservative treatment of Amphotericin B. In four cases which showed very poor vision (0.02-0), surgical treatment (transfrontal craniotomy) was performed and the thickened arachnoid was removed around the chiasma.
