I．緒言

　斜視の手術時視能矯正とはsurgical orthop—tics （手術的視能矯正）とでもいおうか。手術すなわち外眼筋の移動による視能矯正効果をより的確にするために術中両眼視機能の計測と誘発を行ないながら手術を進める方法である。視能矯正法を大別すると手術的方法と非手術的すなわち訓練的視能矯正法がある。手術的方法としては従来から行なわれている斜視手術がこれに相当するが，術中両眼視機能の変化がほとんど実測されておらず視能矯正という点では，やや物足りない感じがあつた。

　手術的視能矯正法の特色としては従来種々の量定法があるが必ずしも常に眼球を良い位置にすることができない。しかしこの方法では術中に眼位をsynoptophoreでたびたび計測するために任意の位置に眼位を調節することが可能で誤差は2°以内である。そして筋移動によつて起こる両眼視機能の変化が手術終了時にわかつているので術後の視能矯正に入りやすい。また手術後の視能矯正に不合理な手術結果を作ることがさけられる。

For the purpose of improving the orthoptic effect by and during the squint surgery, the muscle transposition was carried out following the results of alteration of the deviation and binocularity. The muscle transposition was undertaken tentatively according to the estima-ted calculation first, then detail adjustments were given by the synoptiscopic information.The surgery was performed under 0.2cc of the subconjunctival procaine anesthesia and the synoptiscopic examination on deviation and bino-cularity and short training were given at every muscle transposition.
