白内障手術後に発生したEpithelial downgrowthの治療

The Treatment of Epithelial Downgrowth into the Anterior Chamber Following Cataract Surgery

I．緒言

　白内障手術後の合併症としての前房内上皮侵入Epithelial downgrowthは，比較的稀な合併症であるが，一旦発生すると非常に難治であり，予後の悪いものと考えられている。しかし私共は最近，白内障手術後発生したEpithelial down—growthの症例4例に外科的処置を，1例に外科的処置とX線照射を試み，好成績を収めた。本症を早く診断し適当な処置を行えば，従来考えられていたよりも，予後が意外に良いと考えるので，報告する。

The paper reports the method and result of operation for epithelial downgrowth after cataract extraction. Five cases including one severe one were treated and all cases were cured. The early cases where epithelialzation had not reached the pupillary zone obtained good vision.

Main points in this surgery are :

1) removal of the epithelium from the back of the cornea by curettage and cauterization of the curetted area with 70% ethylalcohol, within three to four mm. from the limbus.

2) large iridectomy over the entire area where the epithelium extends over the iris.

3) administration of steroids after operation.
