Malignant Glaucoma

緒言

　1869年にGraefe^1）が対緑内障手術後の，前房の完全又は不完全消失を伴つた高眼圧の状態をMalignant Glaucomaと記載して以来数多くの報告がなされている。Malignant Glaucomaは文献上虹彩切除術，管錐術，虹彩嵌置術，毛様体解離術などあらゆる対緑内障手術後にみられ，しかもその発生頻度は2〜4％^2）と比較的高く，本院の最近3年間の発生数も第1表に示す如く，対緑内障手術173例中3例と約1.7％に達し，更に他医による対緑内障手術例を加えると4人5眼となる。

　本症に関しては既に先人により数多く記載されているが，未だすべてが解明されたわけではなく，しかも周知の如く早期に適切なる治療を行なわないと予後は極めて不良である。そこで本報ではMalignant Glaucomaの発生機序と治療について考察を試みた。

Five cases of malignant glaucoma are pre-sented followed by a discussion of its mecha-nism and treatment.

It is postulated that the malignant glaucoma is caused by flow of the aqueous posteriorly into the vitreous.

After surgery for simple glaucoma, the com-plication may occur in eyes that have a wide-spread pupillary and peripheral synechia and too large filtering bleb.

Medical treatment consists of the use of my-driatic-cycloplegic drops supplemented by carbo-nic anhydrase inhibitors and hypertonic agests.
