牛眼に余の隅角穿刺術

The Author's Goniopuncture against Buphthalmos

Ⅰ．緒言

　牛眼の手術はとに角，てこずるものの一つである。最近3〜4歳幼児の陳旧な牛眼で両眼殆んどが失明に頻しているものの治療を依頼されたが，これが中々のてこずりものである。両眼の角膜は広く混濁して血管新生し，中央は小水疱角膜炎で粗面状，眼圧は30〜40mmHg，既に両眼に虹彩切除，虹彩籍置術，管錐術等が施されてあり，なお且つ眼圧が充く，隅角鏡で診るも，辛じてかなり広汎の周縁前癒着が認められた。しかし，幸なことに瞳孔は未だ後癒着なく円形4mmほどあり，前房も余り浅くなく深すぎもしない。今までは某眼科医を転々としてコーチゾン，縮瞳剤等を指図されていたという。不思議にも，この様な角膜の大きい患者が最近4例も，その手術を依頼して来院したのである。

　そこでこれ等にも表題の如き手術を行なつて非常に良好な結果を得ているので，この手古摺つた例に対する手術を説明しようと思う。

Surgery of buphthalmos in its late stage often involves difficulties not only as it is an operation in children, but as the sclera shows staphylomatous bulging with occlusion of the chamber angle. The author advocates his goniopuncture which is a safe, effective and convenient method. As a rule multiple gonio-punctures are performed in the upper limbal part of the sclera by use of a foreign body needle taking advantage of its warp.
