トラコーマの診療に関する二三の問題—東京眼科講習会講演内容（昭30.6.5）

Several Problems Concerning Diagnosis and Treatment of Trachoma

〔Ⅰ〕まえがき

　トラコーマ（以下トと略す）問題は，眼科学の中で最も旧い問題であると同時に，また最も新しい問題であるということができよう。最近における抗生物質の驚異的な発展普及に伴い，トの治療には一大革命がもたらされたが，病原問題をはじめ病理発生，診断，分類などの点では，諸家の研究によりかなりの線は出ていると思われるにも拘らず，未だに何人をも首肯せしめるに足る最後的の解決に到達していない。近年トそれ自身が次第に軽症化する傾向がある上に，抗生物質の普及によつて定型的なトの臨牀像が著しく曲げられ，非定型的な症状を呈するものがますます増加しつつある。これがために日常診療に当つて私共はトの診断に一層困惑させられる結果となり，恐らく数十年の後には我国のトはほとんど消滅するであろうと推測される今日においても，ト診断基準の確立或はト分類の統一などが新しい問題として各方面から要望されている。

　以上の如くト問題は，多くの然も極めて難しい問題を妊んでいるが，その二三の点について少しく卑見を申述べたいと思う。

This study is about the processes of development of spontaneously infected trachoma from conjunctivitis (follicularis) chronica to trachoma using 112 cases of school children with particular emphasis on the differential diagnosis of initial trachoma. The trachoma studied in this article is different from both inoculated or acute trachoma. In many cases it is clinically chronic, but its pathological determination must await further studies.

As Dr. Mac Callan declared trachomatous pannus probably appears in all kinds of tra-choma, but it does not seem to appear in initial trachoma.
