要約　目的：両眼に前部虚血性視神経症が発症した側頭動脈炎の症例の報告。症例：84歳女性が5日前からの右眼視野障害で受診した。糖尿病と高血圧があり，加療中であった。所見：矯正視力は右光覚弁，左1.5で，両眼に乳頭の蒼白化があった。左眼に視野狭窄があった。両眼前部虚血性視神経症の診断で，ステロイドパルス療法を開始した。白血球増加と赤沈亢進があった。右浅側頭動脈の生検で，リンパ球などの炎症細胞浸潤と多核巨細胞を伴う肉芽腫性炎症があり，側頭動脈炎と診断した。発症から4か月後の現在，再発はなく，矯正視力は右0，左1.2である。結論：側頭動脈炎による両眼の前部虚血性視神経症に対してステロイドパルス療法を行い，片眼の視機能を維持できた。

Abstract.　Purpose：To report a case of temporal arteritis who manifested bilateral anterior ischemic optic neuropathy（AION）. Case：An 84-year-old female presented with impaired visual field in the right eye since 5 days before. She had been treated for systemic hypertension and diabetes mellitus. Findings：Corrected visual acuity was no light perception right and 1.5 left. Both eyes showed pallor of optic disc. The left eye showed visual field constriction. Pulsed corticosteroid therapy was started under the tentative diagnosis of AION. She also showed leukocytosis and elevated erythrocyte sedimentation. Biopsy of right temporal artery showed granulomatous vasculitis leading to the diagnosis of temporal arteritis. There has been no recurrence for 4 months until present with visual acuity of no light perception right and 1.2 left. Conclusion：Early treatment with systemic corticosteroid resulted in preservation of vision in one eye in a case of temporal arteritis with bilateral AION.
