側頭動脈炎による前部虚血性視神経症

A case of temporal arteritis with anterior ischemic optic neuropathy

　左前頭部から側頭部にかけての頭痛および発熱を伴う側頭動脈炎によると思われた右眼前部虚血性視神経症を発症した49歳女性に対し，副腎皮質ステロイド薬（ステロイド）の投与を早期から行った。右眼は視力低下と水平半盲を残したが，左眼の発症はステロイド投与により予防することができた。本邦において過去に報告された側頭動脈炎による前部虚血性視神経症10例を合わせて検討すると，片眼のみの視力障害でおさまっていた症例では，ステロイドが早期から投与されていた。高齢者で側頭動脈炎によると思われる前部虚血性視神経症をみた場合，重篤な視力障害の両眼発症を防ぐため，片眼発症後早期にステロイド投与を開始することが重要である。

A 49-year-old female developed headache and fever as manifestations of temporal arteritis.Four days later, horizontal inferior hemianopia led to the diagnosis of anterior ischemic optic neuropathy in her right eye. Systemic prednisolone treatment prevented further progression of optic neuropathy. The left eye has remained intact. Review of ten similar cases in Japan has shown that early treatment with systemic corticosteroid prevented involvement of the fellow eye.
