当院におけるインシデントレポートの分析

Review of incident reports in our institution

要約　目的：当院のインシデント・アクシデントレポートを用いて眼科領域の傾向を分析し，今後の課題を明確にする。対象と方法：2007年4月から5年間における，当院眼科に関連するインシデント・アクシデントレポートについて分析した。結果：総報告件数は926件で，職種別報告件数は，看護師が790件（87％），行為別報告件数は，与薬362件（40％）が最も多かった。左右眼に関する報告は42件（4.6％）だった。結論：左右眼の間違えが眼科での特徴である。今回の報告では，看護師間と医師・看護師間での報告が多数を占めた。

Abstract.　Purpose：To present a review of incident reports in our institution. Method：We reviewed 926 incident reports during 5 years through 2012 at the Department of Ophthalmology, Showa University Hospital. Results：Incident reports were presented from nurses in 790 instances（87％）. Errors in medication were reported in 362 instances（40％）. Error in laterality of the eye was reported in 42 instances（4.6％）. Conclusion：Error in laterality of the eye was a characteristic feature in incident reports from the department of ophthalmology. Less than satisfactory nurse-to-nurse and physician-to-nurse coordination was another major finding.
