水晶体皮質が融解・自然吸収した1例

A case of phacolysis with spontaneous absorption

要約　目的：炎症所見を伴わない水晶体皮質融解の症例の報告。症例：62歳女性が内科でWegener肉芽腫症と診断され，精査のため紹介され受診した。所見と経過：矯正視力は右1.0，左指数弁で，眼圧は正常範囲にあった。左眼では水晶体皮質の大部分が消失し，水晶体囊と茶褐色の核のみがあった。前房に炎症所見はなかった。水晶体乳化吸引術，水晶体囊切除，眼内レンズの囊外固定を行い，6か月後に0.6の視力を得た。眼底に異常所見はなかった。結論：無症状の水晶体皮質融解に対して水晶体再建術を行い，良好な結果を得た。

Abstract.　Purpose：To report a case of resolution of lens cortex without signs of inflammation. Case：A 62-year-old female was referred to us under the diagnosis of Wegener granulomatosis. Findings：Corrected visual acuity was 1.0 right and counting fingers left. Intraocular pressure was within normal range in either eye. The left eye showed absence of lens cortex with intact lens capsule and brownish discolored nucleus. No sign of inflammation was present in the anterior chamber. The left eye was treated by phacoemulsification and aspiration, dissection of lens capsule, and out-of-the-bag fixation of intraocular lens. No abnormal findings present in the fundus. The left eye achieved visual acuity of 0.6 six months later. Conclusion：Fair outcome was achieved after surgery in an eye with silent phacolysis.
