両眼先天性水晶体欠損に対して水晶体乳化吸引術および眼内レンズ挿入術を施行した1例

A case of bilateral lens coloboma treated by phacoemulsification-aspiration and intraocular lens implantation

34歳男性が1年前からの視力低下と霧視で受診した。生来近視であったという。裸眼視力は両眼とも0.4で矯正不能であった。水晶体の下方約120°に及ぶ切断状欠損が両眼にあり，軽度の白内障があった。両眼に水晶体乳化吸引術と眼内レンズ挿入術を施行した。下方のチン小帯が疎で脆弱であることに留意して円形前囊切開を行った。水晶体切除の後，水晶体囊拡張リングを囊内に挿入して囊を拡張保持して眼内レンズを囊内に固定した。右1.2，左1.0の術後視力を得た。水晶体欠損眼での白内障手術で水晶体囊拡張リングが有用であった症例である。

A 34-year-old male presented with decreased visual acuity since one year before. His corrected visual acuity was 0.4 in either eye. Both eyes showed a notched defect in the lens involving 120 degrees in the inferior sector. Slight cataract was also present. Both eyes were treated by phacoemulsification-aspiration and intraocular lens（IOL）implantation. We paid due attention to weakness and reduced number of zonules during continuous circular capsulorrhexis. IOL was inserted after lens capsule was dilated by a capsular tension ring. The surgery resulted in visual acuity of 1.2 right and 1.0 left. This case illustrates the usefulness of capsular tension ring during surgery for lens coloboma.
