要約　目的：メチシリン耐性黄色ブドウ球菌（MRSA）による内因性ぶどう膜炎の症例の報告。症例：49歳男性が2型糖尿病のためインスリン治療を受けていた。一酸化炭素中毒のあと，高気圧酸素療法と副腎皮質ステロイドのパルス療法を中心静脈栄養（IVH）下で開始した。悪寒・発熱と白血球増加があり，94日後にIVHカテーテルが除去された。カテーテル先端からMRSAが検出され，その2日後に左眼の眼痛と視力障害が生じた。所見：左眼視力は指数弁で，前房蓄膿と虹彩ルベオーシスがあった。超音波検査で発見された網膜剝離に手術を行い，採取した硝子体からMRSAが検出され，これによる内因性眼内炎と診断した。患眼は眼球癆となった。結論：抵抗性が減弱した状態では，MRSAによる内因性眼内炎がIVHに続発することがある。

Abstract.　Purpose：To report a case of endodphthalmitis caused by methicillin-resistant Staphylococcus aureus（MRSA）. Case：A 49-year-old male had been receiving insulin injection for type-2 diabetes mellitus. After an episode of carbon monoxide poisoning,he started receiving hyperbaric oxygenation and pulse therapy of corticosteroid under intravenous hyperalimentation（IVH）. He showed signs of systemic infection 94 days after start of treatment necessitating removal of IVH catheter. MRSA was detected from the tip of the catheter. He noticed pain and impaired vision in the left eye 2 days later. Findings：Visual acuity in the left eye was counting fingers. The left eye showed hypopyon,rubeosis iridis and retinal detachment by echography. MRSA was cultured from the vitreous obtained during surgery. The left eye became phthisic later. Conclusion：This case illustrates that endophthalmitis due to MRSA may develop following IVH in compromised cases.
