内因性細菌性眼内炎の3例

Three cases of endogenous bacterial endophthalmitis

　内因性細菌性眼内炎3例4眼を経験した。感染源は肝膿瘍が1例にあり，他の2例では不明であった。素因として糖尿病が1例にあった。全例に硝子体手術を行った。採取した硝子体または前房から，1眼にEscherichia coli,1眼にStaphylococcus aureus,2眼1例にKlebsiella pneumoniaeが検出された。手術後に3眼が眼球癆になり，他の1眼では視力が0.04に回復した。前房蓄膿を伴う急性進行性眼内炎をみた場合には，明らかな既往歴がなくても，内因性細菌性眼内炎の可能性を考えて，全身検索と早期の診断的硝子体手術を検討する必要がある。

We observed endogenous bacterial endophthalmitis in 4 eyes of 3 patients. Liver abscess was found as the causative lesion in one case. Diabetes mellitus had been present in another case. All the eyes underwent vitrectomy. From the vitreous or aquous specimen, Escherichia coli was isolated in 1 eye, Staphylococcus aureus in 1 eye, and Klebsiella pneumoniae in 2 eyes of one case. Three eyes resulted in phthisis bulbi and one eye could be saved with the final visual acuity of 0.04. These cases illustrate the necessity of early systemic examinations and diagnostic vitrectomy in cases of rapidly progressive endophthalmitis with hypopyon.
