要約　目的：ドライアイに対する涙点プラグの治療効果の報告。対象と方法：点眼治療が奏効しないドライアイ45例89眼を対象とした。男性25眼，女性64眼で，平均年齢は75歳である。アテロコラーゲン涙点プラグ（キープティア^®）を涙点に挿入し，6か月間の経過を評価した。結果：挿入2週間後に，自覚症状とすべての検査所見が軽快した。Schirmer Ⅰ法の所見改善は2週間後のみが有意で，涙液層破壊時間の延長効果も2週間後をピークに，次第に減弱した。フルオレセイン染色による他覚的所見の改善は，挿入2か月以降も持続していた。自覚症状改善効果持続期間の中央値は4か月であった。治療効果は軽症のドライアイで顕著であったが，重症の21眼にも一定の効果があった。一過性の流涙，眼脂，瘙痒感が計8眼（9％）に生じた。結論：ドライアイヘの涙点プラグ（キープティア^®）は有効で安全であった。

Abstract.　Purpose：To report the therapeutic effect of atelocollagen plug of lacrimal punctum（Keeptear^®）for dry eye. Cases and Method：This prospective study was made on 89 eyes of 45 patients with dry eye that failed to respond to topical medication. The series comprised 25 male and 64 female eyes. The age averaged 75 years. Affected eyes were inserted with lacrimal punctum plug（Keeptear^®）and were followed up for 6 months. Results：Subjective symptoms and all objective findings improved 2 weeks after insertion. Findings by Schirmer I method improved after 2 weeks only. Elongation of tear breakup time reached its peak 2 weeks after insertion. Improvement of subjective symptoms and findings by fluorescein staining lasted after presumed extrusion of the plug 2 months after insertion. Improvement of subjective symptoms lasted for 4 months as the median. Eight eyes（9％）developed transient lacrimation,eye discharge,or itchiness. Conclusion：Occlusion of lacrimal puncta using atelocollagen plug（Keeptear^®）was safe and effective for dry eye.
