Abstract. Purpose:To report a case of advanced keratoconjunctival squamous cell carcinoma manifesting leukoplakia and milky discharge. Case:A 71-year-old male presented with conjunctival hyperemia,discharge,and blurring in the right eye. He had a history of trauma and four surgeries 19 years before. He was in poor general status due to gout with renal failure,pulmonary hypertension,and pulmonary silicosis. Findings:Corrected visual acuity was 0.01 right and 0.5 left. The right eye showed massive milky discharge and a giant tumor simulating cauliflower on the bulbar conjunctiva with leukoplakia,and gelatinous proliferation on the cornea. Biopsy led to the diagnosis of invasive squamous cell carcinoma of bulbar conjunctiva. The tumor decreased in size after radiotherapy totalling 60Gy. The patient died 4 months after the initial visit due to complications of common cold. Conclusion:The present case illustrates that leukoplakia with milky discharge may be the outstanding clinical manifestations in advanced conjunctival squamous carcinoma.
