混合性結合組織病に合併したサイトメガロウイルス網膜炎の1例

Cytomegalovirus retinitis in a case of mixed connective tissue disease

要約　目的：混合性結合組織病の症例にサイトメガロウイルス網膜症が発症した報告。症例：47歳女性が2週間前からの左眼視力低下で受診した。5年前から混合性結合組織病に対して免疫抑制薬と副腎皮質ステロイドで加療中であった。所見：矯正視力は右1.2，左0.2であり，両眼に網膜出血と顆粒状の滲出斑があり，網膜血管炎が疑われた。前房水からサイトメガロウイルスが検出され，ガンシクロビルの全身投与を開始した。網膜血管炎は鎮静化したが，初診から10週間後に網膜剝離が発症し，硝子体手術を行った。左眼に黄斑と視神経萎縮が生じ，初診から23か月後の現在，右1.0，左0.02の視力を維持している。結論：免疫抑制薬または副腎皮質ステロイドを投与中に生じた網膜血管炎は，サイトメガロウイルス網膜炎である可能性がある。

Abstract.　Purpose：To report a case of mixed connective tissue disease who developed cytomegalovirus（CMV）retinitis. Case：A 47-year-old woman presented with failing vision in her left eye since 2 weeks before. She had been treated with systemic corticosteroid and immunosuppressive drugs for mixed connective tissue disease during the past 5 years. Findings：Her corrected visual acuity was 1.2 right and 0.2 left. Both eyes showed retinal hemorrhage and exudative patches suggestive of retinal vasculitis. CMV was positive in the aqueous humor. She started receiving systemic ganciclovir followed by improved retinal vasculitis. Retinal detachment developed 10 weeks later necessitating vitreous surgery. The left eye developed macular and optic nerve atrophy. The visual acuity is 1.0 right and 0.02 left 23 months after her initial visit. Conclusion：The present case illustrates that CMV may be the underlying cause for retinal vasculitis following systemic treatment with corticosteroid or immunosuppressive agents.
