ロービジョンケアの初期対応が困難であった1例

A case of low vision with inadequate initial counselling

要約　目的：発言が少なく，コミュニケーションをとりにくいため，視覚障害への初期対応が不十分であった症例の報告。症例：20歳代の男性が網膜変性の診断で紹介され受診した。矯正視力は両眼とも1.2で，夜盲を自覚し，輪状暗点があり，身体障害者手帳の第5級に相当した。2か月に1回の来院時に，職業選択を含めた生活指導をすることにした。経過：当初は自発的な発言が少なく，生活状況の把握が困難であった。初診の6か月後から発言が多くなり，将来の職業について考え，病変が進行することへの不安を語るようになった。これによりロービジョンケアの目標設定が可能になり，生活課題を分析できるようになった。結論：発言が少ない視覚障害者であっても，初期対応として十分なコミュニケーションをもつことで，自発的に生活問題を明確化することが望まれる。

Abstract.　Purpose：To report a visually handicapped case who received inadequate initial counsel due to poor oral communication. Case：A male in the third decade of life was referred to us for retinal dystrophy. He had been conscious of night blindness. Both eyes had visual acuity of 1.2 with ring scotoma. He started receiving counseling every 2 months. Course：He was poorly communicative about his life initially. He started to actively talk 6 months after his initial visit. He told us about his future vocation and about anxiety over progression of his eye disease. Working together with him,we could set a goal for low-vision care and analyze problems of daily living. Conclusion：This case illustrates that visually handicapped person with poor ability of oral expression can get adequate counselling by establishing communication between patient and physician in the initial stage.
