小児および若年者ぶどう膜炎の統計的検討

Review of endogenous uveitis in children and juveniles

要約　目的：小児と若年者のぶどう膜炎を統計的に検索した結果の報告。対象と方法：過去12年間に秋田大学眼科外来を受診した19歳以下の内因性ぶどう膜炎患者を診療録の記述に基づいて検索した。結果：同時期に受診した内因性ぶどう膜炎患者の総数は692例で，うち43例（6.2％）が19歳以下であった。男性18例，女性25例であり，10例が片眼性，33例が両眼性であった。原因は同定不能18例，間質性腎炎ぶどう膜炎（TINU）症候群7例，サルコイドーシス4例，抗核抗体陽性の若年性慢性ぶどう膜炎4例，Behçet病2例，原田病2例，急性前部ぶどう膜炎2例，帯状疱疹2例，若年性関節リウマチ1例，地図状脈絡膜炎1例であった。結論：原因を特定できたものでは，トキソプラズマ症が皆無であることとTINU症候群が多いことが特徴的であった。

Abstract.　Purpose：To report the outcome of statistical review of uveitis in young persons in our institution. Cases and Method：This retrospective study was made on patients of uveitis under 19 years of age who were seen by us during the past 12 years. Results：A total of 692 patients were diagnosed with endogenous uveitis during the period. Forty-three cases（6.2％）were less than 19 years of age. The series comprised 18 males and 25 females. Uveitis was present unilaterally in 10 cases and bilaterally in 33 cases. Uveitis could not be classified in 18 cases. Tubulointerstitial nephritis and uveitis（TINU）syndrome was diagnosed in 7 cases, followed by sarcoidosis 4 cases, juvenile chronic uveitis positive for antinuclear antibody 4 cases, Behçet disease 2 cases, Vogt-Koyanagi-Harada disease 2 cases, acute anterior uveitis 2 cases, herpes zoster 2 cases, and one case each of juvenile idiopathic arthritis and geographic choroidopathy. Conclusion：The present series of uveitis in young persons was characterized by the absence of ocular toxoplasmosis and by relatively frequent TINU syndrome.
