増殖糖尿病網膜症硝子体手術後失明例の検討

Review of visual loss following vitreous surgery for proliferative diabetic retinopathy

目的：増殖糖尿病網膜症に対する硝子体手術後に失明した症例の検討。対象と方法：2003年までの5年間に硝子体手術を行った増殖糖尿病網膜症159例216眼を対象とした。術後に失明した9例10眼と，しなかった150例206眼につき，失明に関与する要因を多変量解析（ロジスティック回帰分析）で検討した。結果：術後失明の原因は，血管新生緑内障5眼，牽引性網膜剝離3眼，視神経萎縮1眼，術後眼内炎1眼であった。術前に虹彩ルベオーシスがあること，再手術を施行したことが，それぞれ術後失明と有意に相関した。結論：増殖糖尿病網膜症に対する硝子体手術後の失明には，術前の虹彩ルベオーシスの存在が関与する。

Purpose：To identify the causes of blindness following vitreous surgery for proliferative diabetic retinopathy. Cases and Method：This retrospective study involved 216 eyes of 159 patients who received vitreous surgery for proliferative diabetic retinopathy during 5 years through 2003. Visual loss occurred in 10 eyes of 9 patients. These cases and the other 206 eyes of 150 cases were subjected to multivariate analysis. Results：Loss of light perception after surgery was due to neovascular glaucoma 5 eyes,traction retinal detachment 3 eyes,optic atrophy 1 eye,and endophthalmitis 1 eye. Postoperative loss of vision was significantly related with presence of rubeosis iridis before surgery and with multiple surgeries. Conclusion：Preoperative rubeosis iridis is a major factor for blindness following vitreous surgery for proliferative diabetic retinopathy.
