急激な経過をたどったグラム陽性桿菌による内因性眼内炎の1例

A fulminant case of endogenous endophthalmitis caused by gram-positive bacillus

要約　目的：急速に発症進行したBacillus cereusによる内因性眼内炎1症例の報告。症例：74歳男性が直腸癌手術の目的で入院した。5日目に経中心静脈栄養カテーテル（IVH）留置が行われ，その数時間後に右眼の眼痛と視力低下を自覚した。翌日の初診時には，矯正視力は右0，左0.7であった。右眼の全眼球炎と診断し，広域感受性抗菌薬の全身と硝子体投与を行った。病変は急速に進行し，発症から7日目に眼球摘出術を必要とした。硝子体からグラム陽性桿菌であるB. cereusが検出された。全身検索で原病巣は発見されなかった。結論：B. cereusは外因性眼内炎の原因になりやすい。内因性眼内炎を起こすことは稀であるが，易感染状態では起因菌になることがあり注意を要する。

Abstract.　Purpose：To report a fulminant case of endogenous endophthalmitis due to Bacillus cereus. Case：A 74-year-old male was hospitalized for rectum cancer. He received tube insertion for intravenous hyperalimentation（IVH）. A few hours later, he noted pain and blurring of vision in his right eye. When seen the following day, his corrected visual acuity was no light perception right and 0.7 left. Under the diagnosis of panophthalmitis, he was given massive dosis of systemic and intravitreal broad-spectrum antibiotics. The infection progressed rapidly, resulting in enucleation of the affected eye on day 7 of disease. B. cereus was isolated from the vitreous. Systemic examination failed to identify the original site of infection. Conclusion：B. cereus may frequently cause exogenous endophthalmitis. It may cause endogenous endophthalmitis as opportunistic infection as in the present case.
