要約　目的：漿液性網膜剝離に対して副腎皮質ステロイドの大量投与が無効であった全身性エリテマトーデス（SLE）症例の報告。症例：51歳女性が左眼の比較中心暗点で受診した。8年前に関節痛と発熱を契機としてSLEと診断され加療中であった。所見と経過：矯正視力は右1.5，左0.5であった。漿液性網膜剝離が右眼の鼻側と左眼の黄斑部にあり，網膜血管炎が併発していた。SLEに続発した網膜症として，それまで内服中であったプレドニゾロンを増量した。2週後に網膜剝離は軽快したが，その3週後に再発したので，ステロイド大量療法を行ったが無効であり，胞状網膜剝離になった。網膜光凝固で軽快し，初診から10か月後の現在まで再発はない。結論：SLE患者に生じた漿液性網膜剝離では，ステロイドの大量全身投与は慎重に行うべきであり，早期の網膜光凝固が奏効することがある。

Abstract.　Purpose：To report a case of systemic lupus erythematosus（SLE）who developed serous retinal detachment that failed to respond to high-dose corticosteroid. Case：A 51-year-old female presented with relative central scotoma in the left eye. She had been treated for SLE since 8 years before. Findings and Clinical Course：Her corrected visual acuity was 1.5 right and 0.5 left. Serous retinal detachment with retinal vasculitis was present in both eyes. It resolved after increased dosis of peroral prednisolone only to recur 3 weeks later. It failed to respond to pulsed corticosteroid and turned into bullous retinal detachment. Serous retinal detachment resolved after retinal photocoagulation without recurrence until 10 months after her initial visit. Conclusion：Serous retinal detachment in SLE patient may fail to respond to high-dose corticosteroid. Early retinal photocoagulation may be an effective means of treatment.
