Evidence-based clinical pathway for cataract surgery

要約　目的：白内障手術のクリニカルパスがエビデンスとどのように適合しているかの検証。方法：クリニカルパスに含まれる全身検査，涙道通水，術前の抗菌薬点眼，術後点眼，術後内服，術後点滴，眼帯，術後診察間隔についてエビデンスを検索した。ランダム化比較試験，ガイドライン，学会会員アンケート，判例などを参考にした。結果：薬物投与に関しては，術後の抗菌薬点滴以外はエビデンスに従って行われていると思われた。症状や既往がない患者への全身検査は手術合併症とは無関係であり，手術後の診察は術中合併症がなければ翌日は不要で，1，2週間後でよいとするランダム化比較試験があった。結論：白内障手術での全身管理や術後診察は簡略化できる余地がある。

Abstract.　Purpose：To ascertain if the current clinical pathyway for cataract surgery meets the actual evidences. Method：The present study was made on items including：systemic health check,lacrimal sac irrigation,instillation of antibiotics before and after surgery,perioperative medications,eye pad,and interval of postoperative examinations. Following features were also examined：randomized control trial（RCT）,clinical guideline,questionnaire outcomes from peer ophthalmologists,and legal precedents. Results：Perioperative medications met the evidences except intravenous antibiotics after surgery. Systemic examinations in healthy patients was not related to complications related to surgery. RCT showed that examinations on day 2 of uneventful surgery was not necessary,so that the first examination can be done one to two weeks later. Conclusion：The current clinical pathway can be simplified by eliminating systemic examinations in healthy patients and by postponing postoperative examinations.
