要約　目的：硝子体切除と眼内レンズ挿入を同時に行う硝子体トリプル手術で復位した網膜剝離眼での術後屈折値の検討。対象と方法：硝子体トリプル手術を行った裂孔原性網膜剝離15眼と，白内障手術と眼内レンズ挿入のみを行った白内障28眼について，術後6か月以上の時点での屈折値と予測値とのずれを診療録の記述に基づいて検討した。結果：網膜剝離群は白内障群に比べ，平均0.5D（ジオプトリー）近視化していた。近視化は剝離が黄斑に及んでいる6眼で，黄斑に及ばない9眼よりも顕著であった。結論：硝子体トリプル手術で復位した網膜剝離眼では，術後の屈折値が予測値より近視側にずれやすい。これにはガスタンポナーデなどの単一な原因でなく，複合的な要因が関与していると推定される。

Abstract.　Purpose：To compare the actual and predicted refraction following vitrectomy and intraocular lens implantation for rhegmatogenous retinal detachment. Cases and Method：We reviewed 15 eyes of retinal detachment regarding actual and predicted refraction. Another series of 28 eyes operated for cataract served as control. Results：Refraction in the retinal detachment group showed a shift towards myopia by an average of-0.5 diopter when examined 6months or later after surgery. The shift was greater in 6 eyes of retinal detachment involving the macula than the other 9 eyes. Conclusion：Myopic shift may result following simultaneous vitrectomy and intraocular lens implantation for rhegmatogenous retinal detachment. This shift is supposed to be caused not by gas tamponade alone,but by multiple factors.
