要約　目的：裂孔原性網膜剝離に対して行った25ゲージ硝子体手術の成績の報告。対象と方法：過去21か月間に初回手術として25ゲージ硝子体手術を行った網膜剝離74例74眼を対象とした。男性57例，女性17例で，年齢は21～80歳（平均56歳）であった。黄斑円孔，糖尿病網膜症，増殖硝子体網膜症は除外した。全例にSF₆によるガスタンポナーデを行い，水晶体摘出を57眼（77％）に行った。術後観察期間は3～21か月（平均7.6か月）であった。結果：初回手術で70眼（95％），最終的に74眼すべてで復位が得られた。8mmHg以下の一過性低眼圧が8眼（11％）にあった。術前の平均視力0.26が術後0.7に改善した。結論：裂孔原性網膜剝離に対する25ゲージ硝子体手術は有効かつ安全であった。

Abstract.　Purpose：To report the outcome of 25-gauge vitrectomy for rhegmatogenous retinal detachment. Cases and Method：This retrospective study was made on 74 eyes of 74 cases of rhegmatogenous retinal detachment who received 25-gauge vitrectomy during the past 21 months. The series comprised 57 males and 17 females. The age ranged from 21 to 80 years, average 56 years. Cases were ruled out when retinal detachment was associated with macular hole, diabetic retinopathy, or proliferative vitreoretinopathy. All the eyes received additional gas tamponade with sulfur hexafluoride. Lens was removed in 57 eyes（77％）. Cases were followed up for 3 to 21 months, average 7.6 months after surgery. Results：The retina became reattached in 70 eyes（95％）after initial surgery and in 74 eyes（100％）after multiple surgeries. Visual acuity averaged 0.26 before and 0.7 after surgery. Transient ocular hypotony below 8mmHg developed in 8 eyes（11％）. Conclusion：25-guage vitrectomy for rhegmatogenous retinal detachment is safe and effective.
