要約　第3期の特発性黄斑円孔10例10眼に硝子体手術を行った。男性3例，女性7例であり，年齢は58歳から77歳，平均67歳であった。術中に硝子体を可視化する目的で，中央部の硝子体を切除した直後にトリアムシノロン（triamcinolone acetonide）を硝子体腔に注入した。硝子体ゲルに付着したトリアムシノロンを手術顕微鏡で観察した結果，クローケ管は約2乳頭径，硝子体ポケットは約4乳頭径の大きさであった。両者間に薄い硝子体が介在していた。これが眼球運動の際に激しく動揺するときに，硝子体との癒着が強い黄斑部に牽引が及ぶ可能性があると推定された。この機序は，黄斑円孔ないし網膜硝子体界面症候群の発症原因の一部を説明できる。

Abstract.　We performed vitreous surgery for stage 3 idiopathic macular hole in 10 eyes of 10 patients. The cases comprised 3 males and 7 females. Their age ranged from 58 to 77 years,average 67 years. Immediately after core vitrectomy,triamcinolone acetonide was sprayed into the vitreous to visualize the vitreous gel. Triamcinolone adherent to the vitreous was then observed with a surgical microscope. This procedure enabled identification of the inner walls of the vitreous pocket and the canal of Cloquet. The vitreous pocket was about 4 disc diameters and the canal of Cloquet was about 2 disc diameters in size. A thin layer of the viteous was present between the two structures. This thin layer would exert traction to the premacular vitreous cortex when shaken by eye movements. This hypothesis can partly explain the pathogenesis of macular hole and of vitreoretinal interface syndrome.
