過去10年間の先天白内障の術後経過

A ten-year review of surgery for congenital cataract

要約　目的：長崎大学眼科で行った先天白内障手術の成績の評価。対象：1998年までの10年間に手術を行い，術後3年以上の経過を追跡できた先天白内障28例。男性12例，女性16例で，両眼性20例，片眼性8例の48眼である。手術時の年齢は1か月から11歳で平均5.6歳。結果：両眼性20例中4例で両眼ともに1.0以上，11例で0.3以上の最終視力が得られた。両眼ともに0.1以下の最終視力が5例にあり，これらには小角膜症，眼振，緑内障などが術前からあった。片眼性8眼中1眼のみが1.0以上の視力であり，この症例の他眼の視力は0.2であった。他の7例では他眼の視力が良好であり，手術眼の最終視力は不良であった。結論：術前に合併症がない両眼性の先天白内障では，最終視力は比較的良好であった。片眼性では，合併症の有無にかかわらず術後視力は概して不良であった。

Abstract.　Purpose：To review the outcome of surgery for congenital cataract in our department. Cases：The series comprised 28 cases who were operated during 10 years through 1998 and who could be followed up for 3 years or longer. There were 12 males and 16 females. Cataract was bilateral in 20 cases and unilateral in 8,totalling 48 eyes. At the time of surgery,the patients were aged from 1 month to 11 years,average 5.6 years. Result：Out of 20 bilateral cases,4 cases obtained final visual acuity of 1.0 in both eyes. Final visual acuity of 0.3 or over in both eyes was osbtained in 11 cases. In 5 cases,final visual acuity was 0.1 or less in both eyes. These cases had complications including microcornea,nystagmus or glaucoma. Out of 8 unilateral cases,final visual acuity of 1.0 was obtained in one eye only. This case had visual acuity of 0.2 in the fellow eye. The final visual acuity was poor in the other 7 cases who had good visual in the fellow eyes. Conclusion：Good visual acuity was obtained in cases of bilateral cataract without prior complications. Final visual acuity was generally poor in unilateral cases.
