Abstract. We observed epithelial keratitis in 3 cases who were under topical and systemic immunosuppressive therapy. The series comprised a 76-year-old female after two sessions of penetrating keratoplasty,a 46-year-old female after lamellar keratoplasty for corneal dystrophy,and a 74-year-old male after penetrating keratoplasty. All the involved eyes showed large and irregular defects of corneal epithelium with minimal signs of inflammation. The epithelial defect showed daily fluctuations in shape. Additionally,there were findings of dendritic keratitis. These features were suggestive of herpetic origin. All the 3 affected eyes,and not healthy fellow eyes,were positive for HSV-1 DNA in the tear fluid. Topical acyclovir induced cure of corneal lesions. These cases illustrate that epithelial herpetic keratitis is a liability in the presence of irregular and persistent epithelial defects,particular in patients under immunosuppressive therapy.
