　麻痺性兎眼に対して下眼瞼瞼板の水平短縮と下眼瞼形成術を同時に行うKuhnt-Szymanowski法では，高度の下眼瞼下垂と外反がある場合には修正が不十分である。その改善法として，筆者の考案した下眼瞼皮膚切開線を側頭部の外側上方まで延長して外側上方に引き上げ，下眼瞼前葉を眼窩外側縁骨膜にanchor sutureで固定する骨膜固定法（Kuhnt-Szymanowski変法）を4例に行い，奏効した。この方法は特別な材料を必要とせず，高度の下眼瞼下垂と外反がある麻痺性兎眼に対して有効である。

　Modified Kuhnt-Szymanowski method is a standard surgical procedure for paralytic ectropion. It involves horizontal shortening of tarsus of the lower eyelid with plastic correction of the latter. It is unsatisfactory in the presence of severe ptosis of lower eyelid and ectropion. We designed an anchor suture method which involves suturing the lateral portion of lower eyelid to the periosteum with lateral skin excision. It is easy to perform and does not require special items. Excellent outcome was obtained in 4 cases of severe paralytic ectropion by this method.
