眼瞼温存眼窩内容除去術による眼表面悪性腫瘍の治療

Treatment of malignant tumor of ocular surface by orbital exenteration sparing the eyelid

眼瞼と眼球に広く浸潤した眼表面悪性腫瘍に対しては，眼窩内容除去術が標準的な治療法である。この際に眼瞼全層を切除する従来の方法は，侵襲が大きく整容的に大きな問題があり，患者の社会復帰も容易でない。筆者は眼瞼の前葉を温存し，皮膚移植により一期的に義眼床を再建する眼瞼温存眼窩内容除去術を行っている。これによって腫瘍を根治できる一方，整容的にも満足できる結果が得られ，さらに早期の義眼装用ができ術後速やかな社会復帰が可能になった。

Orbital exenteration is the standard procedure for advanced malignant tumor of the ocular surface with extensive infiltration into the eyelid. The widely practised method to excise the eyelid during exenteration results in massive invation,causes aesthetic problems and requires prolonged period before the patients can start social activity. The author's variant of orbital exenteration leaves the anterior palpebral layer intact and includes one-stage reconstruction of the eye socket by skin graft. This method enables radical resection of the tumor,results in fair aesthetic outcome,allows early fitting of prosthesis,and induces early return to professional and social activity.
