　92歳の女性が，4年前に始まり，次第に悪化する右下眼瞼の腫瘍，びらん，出血，疼痛で受診した。糖尿病，慢性腎不全があり，貧血，高血圧，尿路と呼吸器感染症などが併発していた。右眼は眼脂が強く，下眼瞼が膨隆し，眼瞼縁に潰瘍があった。画像診断で腫瘤は下眼瞼全体に広がり，大きさは25×40×27mmで，一部眼窩内に浸潤していた。皮膚科での生検で脂腺癌と診断された。高齢であり，全身状態が不良なので，全身麻酔下での下眼瞼全体を含む拡大切除と眼瞼再建術を行わず，放射線治療を単独で実施することにした。直径6cmの電子線一門で60 Gyを照射し，その終了後2か月で病変部の潰瘍はなくなり，腫瘍は扁平化した。脂腺癌に対する放射線治療の有効性を示す1例であった。

　A 92-year-old woman presented with an enlarging tumor of right lower eyelid since 4 years before. The lesion also showed erosion,hemorrhage and pain. She had diabetes mellitus,chronic renal failure,anemia and systemic hypertension. Diagnostic imaging showed the tumor invaded the whole lower eyelid with some extension into the orbit. Its size was 25×40×27mm. Biopsy led to the diagnosis of sebaceous gland carcinoma. Radical resection under general anesthesia appeared to be too risky because of her age and poor medical condition. She was treated by radiation using 60 Gy of electron beam. The tumor responded to treatment and flatteued 2months after the end of the session. This case illustrates that radiotherapy may be effective for sebaceous gland carcinoma of the eyelid.
