眼窩に発生した末梢性未分化神経外胚葉性腫瘍の1例

A case of peripheral primitive neuroectodermal tumor of the orbit

　7歳女児が1か月前からの右眼の眼瞼腫脹で受診した。矯正視力は右0.04，左1.2であった。右眼は耳側方向に突出し，上眼瞼鼻側に腫瘤を触知でき，眼底に乳頭浮腫と血管拡張があった。磁気共鳴画像検査（MRI）で右眼窩の筋円錐上方に腫瘤があった。骨浸潤はなかった。横紋筋肉腫を疑い，腫瘍を全摘出した。腫瘍は40mm×30mm×25mmの大きさで，多結節状で軟らかく，割面は白色，充実性であった。免疫組織化学染色で上皮性，間葉系，神経系のマーカーすべてが陽性であり，未分化な神経原性の悪性腫瘍と診断した。摘出から3週間後に腫瘍が再発した。化学療法と放射線照射で腫瘍は消失し，1.0の矯正視力を得た。摘出組織の再検討から，末梢性未分化神経外胚葉性腫瘍（peripheral primitive neuroectodermal tumor）の診断が確定した。この腫瘍の眼窩原発例の報告は国内にはなく，海外に9論文がある。

　A seven-year-old girl presented with swelling of right eyelid since one month before. Her corrected visual acuity was 0.04 right and 1.2 left. The right eye protruded laterally. A tumor was palpated upper nasal to the eyeball. Funduscopy showed swollen optic disc and dilated retinal vessels. Magnetic resonance imaging（MRI）showed a tumor superior to the muscle cone in the right orbit. It was excised under the tentative diagnosis of rhabdomyosarcoma. It was 40mm×30mm×25mm in size and was multinodular and soft. Immunohistochemically,it was positive for ectodermal,mesenchymal and neural markers,suggesting undifferentiated malignant tumor of neural origin. The tumor recurred 3 weeks after surgery. Chemotherapy and irradiation led to cure with visual acuity of 1.0. Further histological studies established the diagnosis of peripheral primitive neuroectodermal tumor（pNET）. To the best of our knowledge,only nine case have been reported of this tumor originating in the orbit.
