頭蓋内に進展した視神経膠腫の成人例

Optic nerve glioma with intracranial extension in an adult

　43歳の女性に初発した視神経膠腫の1例を経験した。片眼の視力低下を初発症状とし，その眼の視神経乳頭浮腫と画像上片眼性眼球後方の視神経膠腫を認め，増大傾向を示したため生検を行い視神経膠腫の診断を得た。その後も腫瘍が増大し頭蓋内に進展したため経頭蓋腫瘍全摘出および眼球摘出術を行い，組織学的に腫瘍は完全切除された。術後9か月の時点で再発はみられず，他眼の視機能は正常に保たれている。

A 43-year-old female presented with loss of vision in the left eye. Funduscopy showed disc swelling. Magnetic resonance imaging showed a tumor of the left optic nerve. Biopsy led to the diagnosis of astrocytoma. As the tumor extended into the cranial cavity, the optic nerve and the eyeglobe were surgically removed by transcranial approach. There has been no signs of recurrence during the postoperative 9 months.The right eye has been intact.
