半導体レーザーと鼻内視鏡を用いたDCR鼻外法術後再閉塞例への再建術

Use of diode laser and nasal endoscope in the reconstruction of lacrimal passage after failed dacryocystorhinostomy

　鼻外法による涙囊鼻腔吻合術後の再閉塞5名6例に対して再建術を行った。鼻内視鏡下で半導体レーザーを用いて閉塞部を切開し，NS-tubeを留置する方法である。術後1年以上の観察で，涙道の開存が全例で得られた。本方法は，皮膚を再切開して行う鼻外法による再建よりも侵襲が小さく，閉塞部の同定も容易であった。さらにレーザーを使うことで術後の肉芽瘢痕形成による再閉塞が予防できたと評価される。

　We treated 5 cases of nasolacrimal obstruction after failed dacryocystorhinostomy. One case was bilaterally affected. We used a diode laser to dissect the occluded site under direct observation by a nasal endoscope. An NS-tube was inserted to secure lasting patency of lacrimal passage. After follow-up for one year or longer,cure was obtained in all the cases. Our surgical modality is less invasive than the conventional dacryocystorhinostomy by nasal approach. It is easy to identify the occluded site. Use of laser would prevent later occlusion by suppressing granulation and scarring.
