　手術患者の腰椎X線像，CT，MRIから似たタイプの晒状骨を基に，20mlのディスポ注射筒を用いて各人のオーダーメイド的な円筒形レトラクター(TR)を作製し約70例の経験した後，金属製で径17mmの汎用のTRを作製した．これにより小切開で筋肉などの介在物の侵入を防ぎ，良好な視野で顕微鏡下手術ができるようになった．1椎間当たりの出血量はヘルニアで10.5ml，狭窄症で13.1mlであった．手術時間は両者とも約100分であった．TR法の対象疾患を腰椎分離症の根症状，胸椎黄色靱帯骨化症，高齢者の遅発性脊髄麻痺，頚髄症の一部などに広げている．

　The shorter the distance between the hand of the surgeon and the affected part is, the safer the operation becomes.　In order to achieve less invasive microscopic spinal surgery, we prepared a tubular retractor (TR) that allows close contact with the posterior surface of the lamina and whose length can be adjusted based on the X-P, CT and MRI findings in the patient and dry bone of spine which has resemblance to the patient.
