 To determine the importance of C4-anterolisthesis in cervical myelopathy in aged patients, we studied lateral and dynamic X-rays of 238 in their 20s to 80s. We measured the range of vertebral movement. We studied the frequency of ① antero and ② retrolisthesis of the vertebrae and ③ the inclination angle of the C5-vertebral body to a horizontal line. We calculated ④ the proportion of mean angulation (C1-5) to (C1-7). We studied the correlation between ③ and ④, ① and ③, ② and ③, ① and ④, ② and ④. Results:1)The maximum range of vertebral movement shift from C5-6 to C4-5 occurred in those over 40 years old. 2)Data on ①, ③, and ④ rose in those in their 70s. 3)③ and ④ correlate at all ages. 4)① and ③ and ① and ④ correlate in those in their 70s. 5)No correlation was seen between ② and ③ or ② and ④. In aged patients, the C5-7 vertebral movement range decreased, cervicodorsal kyphosis increased, and hyperlordosis of C1-5 was seen, resulting in posterior rotation of the head and C4-anterolisthesis.
