胸椎黄色靱帯骨化症に対する円筒型レトラクターを用いた顕微鏡下手術（TR法）の検討

Microscopic Excision Using a Tubular Retractor for the Ossification of Yellow Ligament of the Thoracic Spine

　胸椎の黄色靱帯骨化(以下，OYL)を主病変とする胸髄症に対して，胸椎の後方支持組織を温存するため，症例を選び円筒型レトラクター(以下，TR)を用いた顕微鏡下手術(TR法)を行った．症例は10例12椎間，1椎間あたりの平均手術時間は127分，出血量は14mlであった．11点満点での日本整形外科学会頚髄症治療成績判定基準による平均改善率は61.5％であった．TR法は肥厚型までは十分対応可能であり，限局性のOYLに対しては有効な低侵襲手術と考える．

　In order to achieve less invasive microscopic excision for the ossification of the yellow ligament of the thoracic spine, we developed a homemade tubular retractor.　In 10 patients, operation was performed for 12 segments.　The mean operation time was 127 minutes, and the bleeding volume was 14 ml per segment.
