　線維筋痛症（FM）に類似しているがFMの診断基準を満たさない診断不能の慢性痛にアミトリプチリンを投与すると疼痛が消失し，治療中止後1年1カ月が経過しても疼痛は消失したままであった．アミトリプチリンは頭痛を除く神経因性疼痛に最も有効な薬物と考えられている．しかも，日本で使用できる薬物の中でFMに有効である証拠の最も強い薬物でもある．FMに類似の疼痛を訴える患者が通常の治療に反応しない場合には，FMと同じ治療が選択肢となり得る．FMの治療に精通していれば，診断不能の慢性痛も治療可能である．

　A 74-year-old man with unexplained chronic pain analogous to fibromyalgia (FM) did not meet diagnostic criteria for FM but responded to amitriptyline and his pain has disappeared. He did not suffer from pain one year and one month after cessation of treatment. Amitriptyline is thought to be the most effective for neuropathic pain other than headache. In Japan, amitriptyline has the strongest evidence of efficacy in FM. Patients with pain analogous to FM who do not respond to conventional treatment have a range of choices. A detailed knowledge of FM treatment, enables unexplained chronic pain to be treated.
