Abstract 文献概要

　36歳女性の側頭葉単純ヘルペス脳炎におけるmagnetic resonance imaging（MRI）とsingle photonemission computed tomography（SPECT）所見の経時的変化を報告する。炎症巣は右側頭葉内側面の皮質に始まり，側頭葉全体に及び，その後内側部分に出血し，最終的には側頭葉全体に空洞形成をみた。この経過が，MRIとSPECTで経時的に明らかとなった。すなわち，MRIではまず炎症巣は，浮腫を反映してT₂強調画像でhigh intensityで，ついで徐々にT₁強調画像でlow intensityへと変化していき，最終的にはT₁強調画像でlow intensity，T₂強調画像でhigh intensityとなっていった。SPECTでは初めに側頭葉を中心として広範囲にhot spotがみられ，その後このhot spotは側頭葉外側部分に限局し，最終的には側頭葉は広範囲にcold spotとなった。SPECTでみられたhot spotの原因としては，acetazolamide負荷で，cold spotになっていることより，vasoparalysisによるhyperemiaによるものであると考えられた。

We report a case of herpes simplex encephalitis in which the patient was repeatedly examined by magnetic resonance imaging (MRI) and single photon emission computed tomography (SPECT)The patient was a 36-year-old woman who had been transferred to our institution 6 days after the onset of symptoms with mild consciousness distur-bance, nuchal rigidity, and high fever. Cerebrospinal fluid examination revealed an elevated mononuclear cell count with normal sugar concentration. Intra-venous aciclovir was started 7 days after the onset of symptoms.
