　49歳男性。1年前から作業能率が低下，逸脱行為が出現し入院。児戯的，多幸的，表面的で性的発動性の亢進を認めた。WAISは言語性IQ 94，動作性IQ 79，総合IQ 87であった。血液・尿一般，血液生化学検査などに異常はなかった。EEG，頭部CT，MRIで異常はなかったが，SPECTで両側側頭・前頭葉の集積低下を認めた。髄液検査で細胞数，蛋白，C1の増加を認めた。血清，髄液で単純ヘルペスウイルス1型IgG，麻疹ウイルスIgGが上昇し血清／髄液抗体比はそれぞれ12.5，4.75で抗体価指数は8.42，22.22あった。単純ヘルペスウイルス1型と麻疹ウイルスによる慢性脳炎と考えられた。SPECTは病巣の描出に有用であった。人格変化を呈し慢性に経過する症例に慢性ウイルス脳炎を考慮する必要性を示唆した。

In a healthy 49-year-old man, a decrease in job efficiency was noticed along with bizarre behavior. On admission, he was euphoric, childish, superficial and had increased libido. Neurological findings were normal. There were no abnormal findings on routine blood tests, hematochemistry or urinanalysis. MRI showed no abnormal findings. However, single photon emission CT (SPECT) showed diffuse hypoaccumulation of tracer from the temporal to frontal regions. Lumbar puncture showed clear cerebrospinal fluid (CSF) with pleocytosis and an elevated protein level.
