　頸部内頸動脈閉塞でもやもや病様の側副血行路を認め，頭蓋内出血で発症した2症例を経験した。症例1は44歳の男性で脳室内出血（IVH），症例2は58歳の女性で脳動脈瘤を伴わないくも膜下出血（SAH）を示し，いずれも頭痛と意識障害で発症した。2症例とも限動脈を介する外頸—内頸動脈側副血行路を認め，その他に症例1では脳梁背側動脈から脳梁周囲動脈への吻合路とその周辺にもやもや病様血管を，症例2では視床穿通動脈と後脈絡動脈周辺にもやもや病様血管が見られた。出血の原因は発達した側副血正行路の破綻と推察されたが，同様の病態でなぜ一方ではIVH，他方でSAHを示したかは不明である。両症例とも脳血流の低下を認め，血行再建術を行った。CBF study上の改善が得られ虚血症状の予防が期待できたが，再出血の予防ができたかについては長期追跡が必要と考えられた。

We report on two peculiar cases of intracranial iemorrhage due to the rupture of moyamoya dis-ease-like vessels associated with unilateral internal carotid occlusion at its origin. The first case is 44-year-old male showing intraventricular hemorrhage associated with right internal carotid occlusion. The second case is 58-year-old female presenting sub-arachnoid hemorrhage associated with left internal carotid occlusion. Although both cases showed the unique appearance of collateral flow resembling moyamoya disease, they were not classified as Moyamoya disease nor unilateral Moyamoya-like state.
