I．緒言

　われわれは脳血管障害，特に脳卒中の患者に対してroutineに4—vessel angiographyを施行し，すでに1107例に及んでいる。それらについての分析は機会ある毎に発表してきた^14）。この症例中に椎骨動脈が2分し窓形成を示している10例を見出した。興味あることには，そのうち6例に脳動脈瘤を合併しており，臨床症状および血管撮影上小さな脳動静脈奇形の疑われるものが2例あつた。われわれの1107例の4—vessel angiographyのうち，脳動脈瘤は64例74個，脳動静脈奇形8例である。この事実からみると，椎骨動脈に窓形成を示すものの脳動脈瘤または脳動静脈奇形の合併率は非常に高いことになる。文献を探してみたが椎骨動脈の窓形成については，まだまとまった報告がないようである。以下この10症例について脳血管像を中心に検討し，窓形成の発生学的立場からの考察を加えるとともに，脳動脈瘤などとの合併の問題についても検討を行ないたいと思う。

The authors studied on 1107 cases of 4-vessel an-giography performed to the patients who had cere-brovascular disorders. Ten cases which vertebral artery separates into two parts and shows window like appearance mostly around the first cervical ver-tebra were found. This was named assumptively "vertebral diastematoarteria". This is suspected to be remnant of the artery in embrionic period and this was discussed from the view point of embryo-logy.

To be much interested, six cases in these ten cases were associated with cerebral aneurysm and two cases of them were associated with small cer-ebral arteriovenous malformation. Hence the per-centage of this complications shows as 80%, the authors added here some literature consideration.
