Ⅰ．はじめに

　脳動静脈奇形に脳動脈瘤を合併した例は，1942年Walsh ＆ King^13）が初例を報告して以来，その報告は少なくない．しかしながら脳動脈奇形，脳動脈瘤の合併に更に他の血管奇形を同時に有する例はきわめて稀である．頭蓋内血管の窓形成（fenestration）に関しての報告はほとんど椎骨脳底動脈領域におけるものであって，この領域以外の脳血管系での報告はきわめて少なく^3，11），特に脳血管撮影でこれをとらえた例は更に少ない^11）．著者らは，右前頭極部の動静脈奇形，前交通動脈瘤，および右中大脳動脈のsphenoidal segment（M₁部）での窓形成を合併するきわめて稀れな症例を経験したので，症例を中心に若干の文献的考察を加えて報告する．

A case associated with multipe cerebral vascular anomalies, which consisted of fenestration of the middle cerebral artery, arteriovenous malformation and aneurysm the anterior communicatinc artery, was reported.

A 48 year-old male has been suffering from the left paralysis and mental disorder after the initial attack of subarachnoid hemorrhage, and the second attack resulted in the deterioration of the symptoms. He was admitted to our clinic on October 28, 1974.
