I．まえがき

　躯幹近位筋が強く侵され，そのために進行性筋ジストロフィー症にきわめて類似する病像を呈するneuroge）nic muscular atrophyは，Wohlfart　＆　Fex^15）（'55），Kugelberg　＆　Welander^9）（'56）等によりはじめてその特異性が正確に把握されるにいたつた。これ以前にも本症と思われる単発例が筋ジストロフィー症としてあるいは脊髄性進行性筋萎縮症として報告され，なかにはBal—let^2）ら（'08）のごとく前角細胞の変化を伴う筋ジストロフィー症として記載したものもあり，Wohlfartもその初期の論文^14）（'42）には進行性筋ジストロフィー症と神経性進行性筋萎縮症の移行型として報告している。

　このような事実からも明らかなように，本症は実に微妙な病像を備えており，ただちに筋原性あるいは神経原性筋萎縮とは断じがたく，近年の神経診断学の進歩によつてはじめてその全貌を明らかにしているものである。Magee^11）はその要点をだいたい次のようにまとめている。①発病は幼年期または思春期。②主として近位筋を対称性に侵す。③進行はごく緩徐。④劣性遺伝性。⑥多くは筋線維束性播搦を伴う。⑥深部腱反射は減弱ないし消失。⑦本症のneurogenic natureは筋生検と筋電図で確認できる。

A 17-year old boy, who showed generalized muscle weakness and atrophy, most pronoun-ced in proximal region. These were spared in the region of the cranial nerves. No other members of the family suffering from neuro-muscular disorders.

When the patient was 3 year old, the parents discovered his difficulty in walking rapidly and in running and climbing steps. The clinical course had been very slowly progressive. The appearance of this patient simulated very much with progressive muscular dystrophy, walking with waddling gait and climbing up himself. The spontaneous fasciculations, how-ever, were found. The patient showed a hypertricosis in four extremities.

Electromyogram and biopsy specimens sho-wed changes typical of lower motor neuron damage. Dystrophic change of muscle fibers and long lasting insertion activity were the unusual findings. The diagnosis of neuro-genic muscular atrophy simulating muscular dystrophy may be adequate for this clinical pictures, but most probable a benign variant of Werdnig-Hof fmann disease.
