  • Abstract
  • Look Inside

We studied peripheral vascular disturbance in 100 cases of cerebral lesion with a portable photoelectric plethysmograph devised for clinical use. Investigation was focussed upon evidence of inequality in amplitute of volu-me pulse obtained from fingertips on both sides.

1. The results of this investigation show clearly that there exists autonomic repre-sentation of peripheral vasomotor regulation in the cerebral cortex. This is detected always as a contralateral vasomotor distur-bance in cases of unilateral carebral lesions. (Lateralization of peripheral vascular response of cerebral origin.)

2. This inequality of volume pulse or laterality appears as vasoconstric-tion when lesions are considered as irritant, and as vasodilatation when lesions seem to be destructive in nature.

These evidences are likely to appear with lesions in the frontal lobe of the cerebrum, namely, in the premotor and motor areas and the orbital sur-face of the cortex. From data ob-tained from lesions in the convexity of the cerebrum, it can be said that the dominant vasomotor regulatory factor is constrictor; whereas in the orbital surface, both sympathicomimic and parasympathi-comimic effects are obtained by irritation.

3. In cases of lesions found in the " basal" part of the brain, a similar response as in cases of cortical lesions is demonstrated.

4. With infratentorial lesion, even if it is located unilaterally, no lateralization of peri-pheral vascular response is elicited.
