抄録 72例の分裂感情病患者の長期経過について検討した。分裂病性症状と感情病性症状が同時に存在するか,あるいは交代して出現するものを分裂感情病と定義した。本論文では主にこれらの患者の病前の諸特徴について報告する。男女比は1:1.8であり,25歳から35歳の間に発病することが最も多かった。全症例の61%は発病時に既婚であった。また比較的高度の学歴と職業レベルを有するものが多かった。分裂感情病に特徴的な病前性格類型はみられなかったが,双極性のものに強力性人格が,そして単極性のものにはメランコリー親和型が多い傾向があった。病前の対人的接触性は大部分の患者(81%)で良好であった。誘発因子が頻繁(76%)に認められた。分裂感情病患者の68%は精神疾患の遺伝負因を有しており,特に家系内に感情病がみられることが多かった。以上検討した分裂感情病の非症候学的特徴はいずれも精神分裂病よりも感情病に類似したものであることが示された。
72 schizoaffective patients were investigated after a mean follow-up period of 25.6 years (minimum 10, maximum 59 years after onset of the illness) as part of the Cologne study of Marneros and coworkers. Schizoaffective disorders are difined as concurrent or sequential presence of schizophrenic and melancholic ormanic symptomatology. The results of the Cologne study showed that not any depressive or euphoric/maniform symptomatology qualifies a schizophrenic syndrome as schizoaffective but only a symptomatology as defined for instance in DSM-III/DSM-III-R (melancholic type of major depressive episode and manic episode).
