Productivity loss due to mental and physical illness, or presenteeism, is a big problem of psychiatry and occupational health. We conducted a cross-sectional study to evaluate the relationship between various items in The Brief Job Stress Questionnaire, stress check, and presenteeism in 2,905 workers. The average productivity loss due to presenteeism was 5.5% in all participants, and 9.7% in the high-stress group. Variables measured by The Brief Job Stress Questionnaire were directly and indirectly related to presenteeism, accounting for 30.5% of productivity loss and an economic loss of 134,000 yen when converted to nominal GDP per capita. In the stress response, lack of vitality, anxiety, and depression were strongly associated with presenteeism. Occupational stress management has the potential to improve labor productivity in the workplace.
