　（目的）ヘパリン投与中止後のトロンビン活性の一過性亢進について，血小板依存性トロンビン生成能を指標として検討した．（方法）急性冠症候群31例（ヘパリン突然中止群：18例，ヘパリン漸減中止群：13例）と健常対照群31例について，そのトロンビン生成能を測定した．（結果）ヘパリン投与前には，急性冠症候群では対照群に比較してトロンビン生成能は有意に亢進していた．急性冠症候群のヘパリン突然中止群ではトロンビン生成能はヘパリン投与中止後3時間で投与中止直後と比較して有意に亢進していたが，漸減中止群では亢進していなかった．（結語）急性冠症候群では，ヘパリン投与前には血小板依存性トロンビン生成能の有意な亢進が認められた．ヘパリン突然中止群ではヘパリン中止後3時間のトロンビン生成能の一過性亢進を認めたが，ヘパリンの漸減中止群では，この一過性トロンビン生成能亢進は認めなかった．

Aims; This study was designed to investigate therebound increase in platelet-dependent thrombin generation after cessation of intravenous heparin in patientswith acute coronary syndromes (ACS).

Method and Results; The subjects consisted of 18patients with abrupt cessation and 13 patients withgradual cessation of heparin. Platelet-dependent thrombin generation (PDTG) was measured before administration of heparin and at the time of 0, 3, 6 and 24 hoursafter heparin discontinuation. The PDTG was greater inpatients with ACS before administration of heparin thanin 17 healthy control subjects. In patients with abruptheparin cessation, PDTG 3 hours after heparin cessationwas greater than that immediately after heparin cessation. In patients with gradual heparin cessation thisrebound was not detected.

Conclusion; This study demonstrates a transient rebound and increase in PDTG observed after abruptheparin cessation, but not after gradual discontinuationof intravenous heparin.
